DPC Transition
Grace Pediatrics is excited to announce that as of August 1st, 2024, we will be transitioning to a direct primary care (DPC) concierge-style practice. This means that our enrolled patients will have direct access to Dr. Han all 7 days of the week, unlimited visits, and more personalized care through a monthly fee, like a subscription, based on your child's age. We will honor all appointments and bill to your insurance as normal until July 31st, 2024.

If you would like to discuss how the changes might personally impact your family, you may call our office to request a meeting with Dr. Han in the evenings.
Medical Record Transfer/ Release Requests
For a smooth transition to your preferred pediatrician, please complete and upload a signed medical record release request to your child's patient portal account and we will send your child's records promptly.

*Please note that immunization records and summaries of your child's visits with Dr. Han will always be available to you through your patient portal account even if you choose to transfer pediatricians.
